It can be setup on a cpc (cost per click), or by cpm (cost per 1000 impressions), and can be changed in real time according to which is getting the best results. It is possible to get the cost of you ads down to a very low cost per action, where you are getting the most targeted ads that you could possible get anywhere.

Given that Facebook charges higher per click costs for ads with a lower cllck-through rate, it is important to display your ad to only people would would be interested in it.
